Pursuant to Alabama's Public Records Law, I hereby request the following records:

All records concerning any requests to view or obtain the birth certificate of Cecil Coleman Minyard, born in October of 1969, including but not limited to any copies of Alabama Dept of Public Health Form HS-457, any written requests seeking copies, any records of visits to the Etowah County Health Department, and any emails, texts, memoranda, or other written communications concerning any such requests.

The requested documents will be made available to the general public, and this request is not being made for commercial purposes.

In the event that there are fees, I would be grateful if you would inform me of the total charges in advance of fulfilling my request. I would prefer the request filled electronically, by e-mail attachment if available or CD-ROM if not.

Hi Nicole, Thanks for getting back to me. To clarify, I am looking for records concerning any request to view the birth certificate (such as any written requests seeking copies, any records of visits made to the Etowah County Health Department in an effort to seek copies, and any emails, texts, or other written communications concerning the *request* to see the birth certificate, specifically). It is my understanding that these records would not fall under Alabama’s vital records statute.

As stated in my first email, applications for vital records are also
confidential as stated in the Rules of the State Board of Health. In
addition, vital records cannot be "viewed"; disclosure of information from
vital records is provided through the issuance of certificates and/or
certificate data to authorized applicants.
