Please include all available segregable materials, including but not limited to, the number of disciplinary referrals, the nature of the infraction, the manner in which the infraction was resolved (fees, punishments, demerits, dismissed, etc.), whether further prosecution was sought, the degree of the infraction (felony/misdemeanor/informal/etc.), and any other available details.
Further clarification can be provided upon request.
The requested documents are being made as part of a journalistic investigation. They will be made available to the general public. This request is not being made for commercial purposes.
In the event that fees cannot be waived, I would be grateful if you would inform me of the total charges in advance of fulfilling my request. I would prefer the request filled electronically, by e-mail attachment if available or CD-ROM if not.
Thank you in advance for your anticipated cooperation in this matter. I would request your response within ten (10) business days.

I would like to clarify that of the requests I currently have pending with your office, this particular request is of most interest to me at the moment.
