Pursuant to the Arizona Public Records Law, I hereby request the following records:
Historical licensing data for all licensed child care providers (active and closed) that were open anytime from 1/1/19-present:
• Provider name or ID
• Provider address
• Latest license issue date and expiration date
• First license issue date
• Any additional provider data that your agency collects
If electronic records from before 1/1/19 are available, I’d be happy to collect them as well.
The requested documents will be made available to the general public, and this request is not being made for commercial purposes.
In the event that there are fees, I would be grateful if you would inform me of the total charges in advance of fulfilling my request. I would prefer the request filled electronically, by e-mail attachment if available or CD-ROM if not.
Thank you in advance for your anticipated cooperation in this matter. I look forward to receiving your response to this request within 10 business days.

Good afternoon,
Thank you for following up. In response to your public records request for
"historical licensing data for all licensed child care providers...," the
Department requested that you narrow the scope of your request by September
12, 2023 (*see* Department's September 5, 2023 Response
attached); otherwise, the request would be denied due to its scope being
overbroad and because fulfilling it, as written, would cause an undue
administrative burden. Because no response was received by that date, the
Department administratively closed your requested as denied for the reasons
stated. Should you choose to narrow the scope of your request a this time,
the Department will review your request with the assistance of its counsel
and respond accordingly.
