Records re: inmate work programs at CACF

Pursuant to the state open records law, Ariz. Rev. Stat. Ann. Secs. 39-121 to 39-122 and 39-128, I hereby request the following records:

The following materials related to Inmate Work Programs at Central Arizona Correctional Facility, as indicated by Sections 11.13 and 11.14 of Contract No. 040176DC:

• All incident reports/notifications related to the occurrence of "medical services for injuries or illnesses incurred by inmates while participating in work programs, on-site" and off-site.
• All on-site work programs
• All inmate time sheets or related invoices/reports
• All list of jobs reports (provided each month), including which jobs have been filled, total inmate wages earned for each job, the average wage earned, Contractor problems and concerns
• Documented notice of Department Monitor concerns related to reviews of the monthly listing of inmate jobs

The requested documents will be made available to the general public, and this request is not being made for commercial purposes.

In the event that fees cannot be waived, I would be grateful if you would inform me of the total charges in advance of fulfilling my request. I would prefer the request filled electronically, by e-mail attachment if available or CD-ROM if not.
