To Whom It May Concern:
Pursuant to the West Virginia Freedom of Information Act, I hereby request the following records:
Informational flyers regarding health concerns are being sent to parents and guardians in Berkeley County, WV under funding by GSK. I am requesting records relating to how the Berkeley County Health Department is given, retrieves, compiles, etc., the database list of recipients, the minor’s name associated and the mailing address used to send these flyers.
Also, all records documenting third parties associated with this list who are able to access this information on this database and what this information is alternatively used for.
The requested documents will be made available to the general public, and this request is not being made for commercial purposes.
In the event that there are fees, I would be grateful if you would inform me of the total charges in advance of fulfilling my request. I would prefer the request filled electronically, by e-mail attachment if available or CD-ROM if not.
Thank you in advance for your anticipated cooperation in this matter. I look forward to receiving your response to this request within 5 business days, as the statute requires.

No documents available have been received by the Berkeley-Morgan County Board of Health 122 Waverly Ct. Martinsburg, WV 25403 in reply to the FOIA for GSK.
Only information is that community members had been notified to contact their local health department to ensure their vaccinations are up to date. This was not a notification that was made by the health department.
