Pursuant to the Massachusetts Public Records Law, I hereby request the following records:

A record of all inspections conducted at the Erich Lindemann Mental Health Center located at 25 Staniford St, Boston, MA between January 2016 and January 2019, including copies of written evaluations.

Please include any annual or routine inspections carried out by state authorities or private contractors. I also request:

-Any contracts with private pest control contractors. (The state's website says DCAMM's operations team oversees private contractors for services like pest control: https://www.mass.gov/service-details/pest-control-mccormack-lindemann-hurley-buildings)

-Please include any Incident Report Forms received between Jan. 2016 and Jan. 2019 regarding the Lindemann building. (A blank copy of the Incident Report Form is included in the Lindemann Building Occupant HandBook: https://www.mass.gov/files/documents/2016/12/rz/lindemann-occupant-handbook.pdf.)

I also request that, if appropriate, fees be waived as we believe this request is in the public interest, as suggested but not stipulated by the recommendations of the Massachusetts Supervisor of Public Records. The requested documents will be made available to the general public free of charge as part of the public information service at MuckRock.com, processed by a representative of the news media/press and is made in the process of news gathering and not for commercial usage.

I expect the request to be filled in an accessible format, including for screen readers, which provide text-to-speech for persons unable to read print. Files that are not accessible to screen readers include, for example, .pdf image files as well as physical documents.

In the event that there are fees, I would be grateful if you would inform me of the total charges in advance of fulfilling my request. I would prefer the request filled electronically, by e-mail attachment if available or CD-ROM if not.

You received your documents on February 20th via email and also via our Interchange File Transfer. I am including here the email with the attachments and if you can look through email for the “You have files” email message you will find that you have a link available to view the files that were too large to send.
