Pursuant to the California Public Records Act (California Government Code §§ 6250), I hereby request the following records:
All evaluations, inspections, reports, and related materials regarding all services, equipment, and activities involving/provided by/contracted for regarding North Fork Correctional Facility and any facilities associated with it, either before, during, or after the contract term for which the facility was under obligation to the California Department of Corrections and Rehabilitation.
Responsive materials should include but not be limited to:
• all annual inspections
• all emergency event reports and evaluations
• all records of inspections conducted in accordance with DOC provisions, such as those of the American Correctional Association
• all reports regarding the transfer of prisoners
• all reports regarding the performance of the contract and associated duties
The requested documents will be made available to the general public, and this request is not being made for commercial purposes.
In the event that fees cannot be waived, I would be grateful if you would inform me of the total charges in advance of fulfilling my request. I would prefer the request filled electronically, by e-mail attachment if available or CD-ROM if not.
Thank you in advance for your anticipated cooperation in this matter. I look forward to receiving your response to this request within 10 calendar days, as the statute requires.

Thanks so much for your message. I'd like to limit this request to all annual evaluations of the facility conducted by the CDCR and any emergency event evaluation reports. Would this sufficiently narrow the results? I'm happy to discuss this further.
