Genetic Counselors data

To Whom It May Concern:

Pursuant to the California Public Records Act (California Government Code §§ 6250), I hereby request the following records:

I would like to request all the data collected about licensed genetic counselors in the state, since the creation of state licensure laws for this profession. When a genetic counselor receives a state license, what information is collected about them? In particular, I hope the data will include how many licensed genetic counselors receive licenses year by year, their names, education, contact information as well as any other information collected. Additionally, I want to know how many counselors have had their licenses revoked and for what reasons.

The requested documents will be made available to the general public, and this request is not being made for commercial purposes.

In the event that there are fees, I would be grateful if you would inform me of the total charges in advance of fulfilling my request. I would prefer the request filled electronically, by e-mail attachment if available or CD-ROM if not.
