* Specific information from this state's prescription drug monitoring program (PDMP) data collection. I am requesting that the data be sent in an excel sheet.
Please note that I am asking for high-level numbers; I have omitted anything that might personally identify a patient.

* Please also include the data dictionary for this PDMP dataset.
* I am also requesting any complaints filed by doctors in the area regarding the PDMP and doctor-patient confidentiality.
* I am also requesting any and all documents, policies, and memos regarding police or DEA access to this database.
* Any privacy safeguards concerning access to this information
* I am also requesting any and all documents, memos, emails, and policies concerning how it was decided to track these specific drugs.

The requested documents will be made available to the general public, and this request is not being made for commercial purposes.
In the event that there are fees, I would be grateful if you would inform me of the total charges in advance of fulfilling my request. I would prefer the request filled electronically, by e-mail attachment if available or CD-ROM if not.
