Pursuant to the California Public Records Act, I hereby request the following records:

1) copies of all IRB applications made by Dr. Vanessa Worsmer or for studies in which Dr. Vanessa Wormser was listed as an investigator;
2) all continuing reviews pertaining to the studies in paragraph 1;
3) all protocol enrollment exceptions pertaining to the studies in paragraph 1;
4) all adverse events, protocol violations, or incidents, including outcome letters, submitted in regard to the studies in paragraph 1;
5) all study closeout reports pertaining to the studies in paragraph 1 ;
6) an "Email activity report" for Dr. Worsmer's email mailbox, covering the longest timespan available in the reporting system;
7) an "Email non-owner access report" for Dr. Wormser's email mailbox, covering the last 180 days of this request.
8) all mailbox audit logs for Dr. Wormser's email mailbox, covering the last 180 days from the date of this request.
9) a "Mailbox content search and hold" report regarding Dr. Wormser's email mailbox;
10) the admin audit log for Dr. Wormser's email mailbox;
11) a mailbox usage report for Dr. Wormser's email mailbox;
12) all duty log reports, as required by the ACGME, for Dr. Wormser for the last 12 months;

The requested documents will be made available to the general public, and this request is not being made for commercial purposes.

In the event that there are fees, I would be grateful if you would inform me of the total charges in advance of fulfilling my request. I would prefer the request filled electronically, by e-mail attachment if available or CD-ROM if not.

This letter is to acknowledge your written request related to documents regarding "...UCSF/ Wormser IRB Approvals and Requests ..." made pursuant to the California Public Records Act (CPRA), Government Code ยง6250 et seq., and dated January 23, 2021.

A search for responsive documents will be commenced for items 1-5 and 12. I will write again in regard to items 6 - 11.

Please note that due to the impact of COVID-19 on UCSF administrative operations (e.g., office closures and/or requiring certain staff to work remotely), responses may take longer than usual. Our estimated date of production for all new requests is currently eight weeks, although we are sometimes able to provide records in a shorter timeframe.

This request is still inexplicably outstanding. The statutory deadline for a full response has passed, and the University once again risks a lawsuit to compel the production of responsive records. Simply ignoring requests for updated status is unacceptable. I would like an explanation and update on the status of the request by the end of business on Monday, April 19, 2021.
