Pursuant to the California Public Records Act, I hereby request the following records:

Any and all contracts, agreements, letters-of-intent, purchase orders, invoices, and billing documents between University of California and Metlife regarding the agreement between said parties described in the attached joint press release from University of California and Metlife.

Any and all written communication, or digital communication including but not limited to letters, email, excel spreadsheets, slide documents discussing said relationship or agreements during the period January 1, 2019 to present.

Any and all documentation of meetings between University of California and Metlife, including, but not limited to recordings, minutes, photographs.

Documentation of all monetary payments or other consideration made or promised by Metlife to University of California.

Count of how many individuals in each category: Staff, Faculty, Students, Alumni, University of California has delivered information to Metlife.

Count of how many individuals in each category University of California has offered or promised to deliver data.

Documentation of what data fields were provided or were promised to be provided by University of California for individuals in any of these categories. For example: Name, Address, Phone Number, Date of Birth, Gender, Marital Status, SSN, Degree obtained, Graduation date, Classes taken, GPA, medical conditions, medical treatments, disciplinary actions, etc.

Metlife may also be known as "Metlife Choice", "Metlife Auto & Home Insurance Agency, Inc.", "Metlife, Inc." or other variations.

University of California may also be known as "University of California Office of the President", or "UCOP", or "University of California, Berkeley", "University of California, San Diego", "University of California, Davis" or other variations.

The requested documents will be made available to the general public, and this request is not being made for commercial purposes.

In the event that there are fees, I would be grateful if you would inform me of the total charges in advance of fulfilling my request. I would prefer the request filled electronically, by e-mail attachment if available or CD-ROM if not.

Thank you in advance for your anticipated cooperation in this matter. I look forward to receiving your response to this request within 10 calendar days, as the statute requires.
