Pursuant to the Illinois Freedom of Information Act., I hereby request the following records:
Email communications between Madeleine Johnson and any teacher or administrator at Franklin Middle School regrading student Miles Shalleneberger, and any paperwork filed by Madeleine Johnson at Franklin on behalf of student Miles Shallenberger.
I would also like to request to include email or other digital correspondence through the Home Access app from Madeleine Johnson regarding Miles Shallenberger. This could be teachers or other school administrators, including but not limited to: Pickell, Joseph Anderson-Bliss, Evan Chancellor, Courtney Harris, Julie Cline, Payton Gerlach, Kaylee Holcomb, Nathan Schulze, Nicholas
I would also like phone or other records related to which guardian called Miles Shallenberger in on days he was tardy or absent from school and what the reason was.

To the extent that these records exist, they are not subject to release
under FOIA because they would be considered Student Records. If you wish to
submit this request as a Student Records Request, you can do so by
completing the attached form and submitting it to the child’s school. Would
you still like us to process this as a FOIA request?

Thanks for this information. Once his dad fills out this form, will we need to attach it here and move forward with the FOIA request? Or will this form compel the school to turn over Miles' full file?

We are suggesting you withdraw your FOIA request (it has to be in writing).
Once the school has received the completed form, it will start the process
for the Student Records Request.
