- A blank Employee Incident Report form.
- All Employee Incident Reports submitted for the Area Manager 5 North Region from January 1, 2024 through August 1, 2024.
The requested documents will be made available to the general public, and this request is not being made for commercial purposes.
In the event that there are fees, I would be grateful if you would inform me of the total charges in advance of fulfilling my request. I would prefer the request filled electronically, by e-mail attachment if available or CD-ROM if not.

Please see the attached documents for the FOIA 5725.
As explained in the attached response letter, your second Request is unduly burdensome and the Park District extends the offer for you to narrow your request for example by providing keywords for the search parameters. If you would like a telephone conference to discuss narrowing your request, please call 312-742-0867.
Under section 140/3(e), the Park District is extending the time to respond to your request by five business day as a courtesy on this FOIA to give you the opportunity to narrow your search. If this is acceptable to you, please respond in writing to agree to this extension. The new due date for your request is Friday August 16, 2024 by close of business.
