Pursuant to the Illinois Freedom of Information Act., I hereby request the following records:
Complete FOIA logs from April 27, 2024 through May 31, 2024 (inclusive of those dates).
At minimum, please include the following:
Date the request was filed
Name of requestor,
status of the request,
Request System assigned identifier such as a Request Number, if applicable
Please provide the record in an excel format. I have attached an equivalent FOIA log that reflects what I am looking for.
The requested documents will be made available to the general public, and this request is not being made for commercial purposes.
In the event that there are fees, I would be grateful if you would inform me of the total charges in advance of fulfilling my request. I would prefer the request filled electronically, by e-mail attachment if available or CD-ROM if not.
Thank you in advance for your anticipated cooperation in this matter. I look forward to receiving your response to this request within 5 business days, as the statute requires.

The Chicago Park District (the “Park District”) responds to all public records requests by the Illinois Freedom of Information Act (FOIA), 5 ILCS 140/1 et seq. Please expect a response by Wednesday July 10, 2024 by close of business. If additional time is needed to assemble and copy all documents responsive to your FOIA request, we will contact you with a time estimate and a request to extend.
Please note that the Park District is closed Thursday July 4, 2024 for the holiday.
