1. All physician disciplinary actions from 2019-2024 related to:
- Mental health conditions
- Substance use
- Behavioral health issues
2. Board policies regarding physician health programs
3. Statistics on:
- Self-reported vs. discovered cases
- Return to practice rates
- Appeal outcomes
4. Any studies or reports on physician mental health programs
5. Details about Colorado Physician Health Program (CPHP):
- Relationship/agreements with medical board
- Confidentiality policies
- Rate of board reporting
- Statistics on physician outcomes
- Any formal or informal agreements between CPHP and board
6. Any evaluations or audits of CPHP effectiveness
The requested documents will be made available to the general public, and this request is not being made for commercial purposes.
In the event that there are fees, I would be grateful if you would inform me of the total charges in advance of fulfilling my request. I would prefer the request filled electronically, by e-mail attachment if available or CD-ROM if not.
