We are requesting death records dating January 1, 2020 to January 1, 2021, for Montezuma county with the following information for each case:

- decedent name
- date of death
- location of death
- race

- gender
- occupation/industry
- whether injury/infection occurred at work
- incident location (street address, city, state, ZIP where injury/infection occurred)
- incident date
- decedent residence (street address, city, state, ZIP)
- cause(s) of death (indicating if COVID related)
- contributing factors
- any other accessible, non-exempt information contained in electronic case records

The requested documents will be made available to the general public, and this request is not being made for commercial purposes.

In the event that there are fees, I would be grateful if you would inform me of the total charges in advance of fulfilling my request. I would prefer the request filled electronically, by e-mail attachment if available or CD-ROM if not.

Good afternoon,

The Department is responding to your request pertaining to: death records
dating January 1, 2020 to January 1, 2021, for Montezuma county with the
following information for each case:

- case number
- decedent name
- date of death
- location of death
- race
- ethnicity

- gender
- occupation/industry
- whether injury/infection occurred at work
- incident location (street address, city, state, ZIP where
injury/infection occurred)

Certificates of death are a vital statistics record pursuant to § 25-2-117,
C.R.S., which states, “vital statistics records shall be treated as
confidential, but the department of public health and environment shall,
upon request, furnish to any applicant having a direct and tangible
interest in a vital statistics record a certified copy of any record
registered under the provisions of this article.”

Additionally, Colorado Board of Health regulations (5 CCR 1006-1) also
state, "The State Registrar or other custodians of vital records shall not
permit inspection of, or disclose information contained in, vital
statistics records, or copy or issue a copy of all or part of any such
record unless he is satisfied that the applicant has a direct and tangible
interest in such record." Those with a direct and tangible interest in
Colorado death certificates (in whole or part) are outlined further on the
vital records website.

The data you have requested is confidential pursuant to Section 25-2-117,
C.R.S. and 5 CCR 1006-1, therefore the Department is not able to provide
the specific data you have requested. We have copied our vital statistics
team here, to assist with providing aggregate data that is responsive to
your request.
