Pursuant to the Georgia Open Records Act, I hereby request the following records:

All records and documents held by the Columbus Police Department regarding a Tesla crash that occurred on 5/21/2023.

According to media reports the crash happened near Hancock rd, Columbus, GA at around 11:22 p.m.

The driver of the Tesla was Phong Do, 54, and the passenger was Toan Neugen, 48.

The records requested include but is not limited to:

Incident report

Investigative records (interview transcripts, scene models)

CAD report

All video and audio recordings (dash cam, body cam, mobile video, interviews, video of the scene)

All photos

Radio dispatch recordings

911 calls

After action reports

The cost estimate stated "Please do not remit payment until the final invoice is sent with requested materials.
Please contact this office with approval of costs associated with this request". On 1/23/24 I emailed and said I accepted the charges and I have not received the final invoice.
