Pursuant to the Texas Public Information Act, I hereby request the following records:
A copy of all autopsy reports performed between 1/1/2010 and the date this request is processed on individuals who died at a Timberlawn Behavioral Health System facility.
The requested documents will be made available to the general public, and this request is not being made for commercial purposes.
In the event that there are fees, I would be grateful if you would inform me of the total charges in advance of fulfilling my request. I would prefer the request filled electronically, by e-mail attachment if available or CD-ROM if not.
Thank you in advance for your anticipated cooperation in this matter. I look forward to receiving your response to this request within 10 business days, as the statute requires.

Does she still wish to do so or should I interpret the request for payment as an indication that that is no longer necessary? Hi there,
Just wanted to follow up on this request. I spoke with Ashley last month and she informed me that due to limitations of the database there may not be any autopsy reports before 2011 or so. I informed her that this is fine. I am wondering how many autopsy reports have been identified so that I can mail a check for the cost.

Thank you for the response. I think there is some confusion here. I don't know how many autopsy reports fall within the time frame of my request, so I don't know how much money to send. Can you inform me how many autopsy reports fall within the time frame of my request?

My apologies for the confusion, there are 3 (three) autopsy reports. Are you wanting non-certified copies or certified copies? The fee is listed below:
Copy of Non-certified autopsy report - $5 x 3 autopsy reports = $15 Or
Copy of Certified autopsy report - $15 x 3 autopsy reports = $45
