Pursuant to the Georgia Open Records Act, I hereby request the following records:
Decatur Police Department Use of Force policy, use of force reporting template/form, any statistical data on use of force compiled by the Decatur Police Department 2010-2015.
The requested documents will be made available to the general public, and this request is not being made for commercial purposes.
In the event that there are fees, I would be grateful if you would inform me of the total charges in advance of fulfilling my request. I would prefer the request filled electronically, by e-mail attachment if available or CD-ROM if not.
Thank you in advance for your anticipated cooperation in this matter. I look forward to receiving your response to this request within 3 business days, as the statute requires.

Dave Mass sent an email on September 6, 2016 requesting documents pertaining to the Decatur Police Department's use of force incidents. In the email, it was asked that if there were any fees then he would want to be contacted first. I emailed Mr. Mass on September 9, 2016 to discuss the fees with him and have not received a telephone call. Please have someone call me so that we can discuss the fees and the Open Records Request can be completed.
