Orchids of Asia spa records

To Whom It May Concern:

Pursuant to the Florida Sunshine Law, I hereby request the following records:

Any and all inspections, orders and violations pertaining to the Orchids of Asia Day Spa located at 103 South US Highway 1 in Jupiter, Florida. To include all reports and photographs relating to a November 14, 2018 inspection carried out by Health Investigator Karen Herzog.

The requested documents will be made available to the general public, and this request is not being made for commercial purposes.

In the event that there are fees, I would be grateful if you would inform me of the total charges in advance of fulfilling my request. I would prefer the request filled electronically, by e-mail attachment if available or CD-ROM if not.

Good afternoon Brad,

Thanks very much for the docs and photos. I just want to double-check that there is nothing still to come.

A publicly accessible police affidavit discussing the November inspection suggests that Inspector Herzog took further photos of beds, sheets and pillows, and dressers containing clothes.

Furthermore it explicitly says that Herzog took photos of IDs/licensed or at least two more people she encountered on the premises.

The document forwarded to me from yourselves only includes images of a refrigerator and an ID/license for one employee, not three.

I wondered therefore if there are additional images that have not been provided to me.
