Pursuant to the California Public Records Act, I hereby request the following records:
Informational copies of all American Indian and Alaska Native death certificates held by the Humboldt County Clerk's office.
The requested documents will be made available to the general public, and this request is not being made for commercial purposes.
In the event that there are fees, I would be grateful if you would inform me of the total charges in advance of fulfilling my request. I would prefer the request filled electronically, by e-mail attachment if available or CD-ROM if not.
Thank you in advance for your anticipated cooperation in this matter. I look forward to receiving your response to this request within 10 calendar days, as the statute requires.

Vital records are not obtainable via public records request.
You may purchase informational copies of death certificates using the application for certified copy of death record form linked here for your convenience. Each informational certificate of death ordered from our office is $21.
Alternatively, you may purchase records on-line using a credit card through our third-party vendor Vitalchek. (convenience fees apply)
Please complete the information requested on the form for each decedent you’d like an informational copy for. At a minimum you must provide the name of the decedent and the date of death (or approximate date of death within 10 years). Please note: No other criteria made part of death certificates, including decedent’s race, is indexed or searchable.
