Pursuant to the Idaho Public Records Act, I hereby request the following records:

I am requesting an opportunity to inspect or obtain copies of public records that include an updated list of 1)all Idaho State Medicaid healthcare providers along with 2) All state licensed Idaho healthcare providers in an electronic data set with inclusion of their associated National Provider Identifier (NPI) numbers and associated Hawaii state healthcare license number. CSV Electronic format is favored, however excel or PDF electronic format would also be acceptable. The list of all Medicaid and state licensed healthcare providers should include Physicians (MD, DO), Physician Assistants, Nurses, CRNA, Nurse Practitioners, Midwives, Medical Assistants, Dentists, Podiatrists, Dental Assistants, Veterinarians, Chiropractors, Physical Therapists, Optometrists, Occupational Therapists, Social Workers, Family therapists, Psychologists, Emergency Medical Personnel (EMTs, Paramedics), Respiratory Therapists and so on. All types of practitioners from all prior dates electronically available would be appreciated (1970-present). Specific items to be included in data set:

1. Provider name
2. National Provider Identifier (NPI) number
3. Idaho State Healthcare License number and initial date of issuance or expiry (if available/applicable)
4. Education / School/Training, if available
5. Provider type (MD, DO, PA, RN, PT, OT, OD, CRNA, DMD, DDS, DPM etc)
6. Specialty
7. Practice Location

If there are any fees for searching or copying these records, please email me if the cost will exceed $50. However, I would also like to request a waiver of all fees in that the disclosure of the requested information is in the public interest and will contribute significantly to the public’s healthcare transparency, fairness and public awareness.
The law requires that you respond to and fulfill this request "as promptly as possible." If you expect a significant delay in responding to and fulfilling this request, please contact me by phone or via email with information about when I might expect copies or the ability to inspect the requested records.
If you deny any or all of this request, please cite each specific exemption you feel justifies the refusal to release the information and notify me of the appeal procedures available to me under the law.

2) Reason for denial.
The Division of Occupational and Professional Licenses (DOPL) is unable to provide the information requested for 1 & 2 - Emergency Medical Personnel, Medical Assistants, Dental Assistants as well as a & b.

1. DOPL does not have access to a list of providers approved by Medicaid. This information may be available through the Department of Health and Welfare.
2. Emergency Personnel are not licensed through DOPL. Dental Assistants & Medical Assistants are not licensed.

1. NPI numbers are accessible online from various sites including NPPES NPI Registry<https://npiregistry.cms.hhs.gov/search>
2. DOPL does not have access to information for the state of Hawaii

In response to 2 - c, d, e, f, g, h. This information is readily available online. Please go to the Division of Occupational and Professional Licenses website<https://dopl.idaho.gov/chi/>. Hover over Occupations & Professions, select the profession you are interested in (Dentistry, Medicine, etc.) You will be taken to that Board's website. From there select "Search for a License". Select the Board, or Profession and/or License type from the drop-down boxes , then select Start Search at the bottom of the screen. This will take you to an alphabetized list of licensees. You can export that data to an excel spreadsheet ("Send to Excel" button at the bottom of the screen). Some Board's provide education/school and specialty, some do not. If it is not provided through the website, it is not available through DOPL.

The Board of Dentistry requires completion of an order form<https://elitepublic.isbd.idaho.gov/IBODPublic/ListPurchaseOrderForm.pdf> and charges a $20 fee for a list of licensees. The fee is established by the Board of Dentistry and is not something DOPL can waive.

The license search for Nurses is a little different so I have attached the lists of Registered Nurses, Licensed Practical Nurses and Advanced Practice Registered Nurses (APRNs - of which CRNA's and Nurse Practitioners are included).

The Board of Veterinary Medicine's site does not provide the option to export their list to Excel. I have attached that list as well.
