Pursuant to the Illinois Freedom of Information Act., I hereby request the following records:

Please provide records and answers for the time period of June 2022 to June 2024.

* Number of complaints regarding the Illinois Department of Corrections sent to your office (monthly and yearly).
* Number of complaints resolved in favor of constituents' requests (monthly and yearly).
* Number of complaints sent back to IDOC (monthly, yearly).
* Number of repeated complaints by a particular constituent (i.e., constituent A filed 27 complaints in 2023), according to month/year.
* Type of complaints submitted to your office, regarding the IDOC (i.e., staff, healthcare, grievances, excessive force).
* Outcome of IDOC complaints handled by your office.
* Length of time for resolution of IDOC complaints sent to your office.
* IDOC complaints by facility, including types (monthly, yearly).
* List of any outside agencies consulted or collaborated with in resolution of IDOC complaints, other than the governor's office or IDOC.
* Number of founded (valid) complaints against IDOC (monthly and yearly).
* Number of rejected (unfounded) complaints against IDOC (monthly and yearly).

The requested documents will be made available to the general public, and this request is not being made for commercial purposes.

In the event that there are fees, I would be grateful if you would inform me of the total charges in advance of fulfilling my request. I would prefer the request filled electronically, by e-mail attachment if available or CD-ROM if not.
