Please provide the records requested below for June 2023 to June 2024.
* Number of complaints received by Constituent Services (monthly and yearly).
* Type of complaints received by Constituent Services.
* Resolution of complaints received by Constituent Services (i.e., investigated, transferred). By type and category, both monthly and yearly.
* Number of employees in Constituent Services (monthly and yearly).
* Employee position/title in Constituent Services (monthly and yearly).
* Names, titles, and direct contact information for each Consituent Services employee (as of June 2024), including extensions.
* Names of agencies and/or organizations consulted or collaborated with, in order to resolve complaints (monthly, yearly).
* Outcome of complaints which were resolved with the assistance of other agencies, offices, and organizations (monthly, yearly).
* Length of time for resolution of complaints (monthly and yearly).
* Number of repeated complaints filed by same constituent (monthly and yearly).
* Number of complaints by facility (monthly and yearly).
* Number of complaints resulting in IDOC employee suspension or dismissal (monthly and yearly).
In the event that there are fees, please inform me of the total charges in advance of fulfilling my request. I would prefer the request be filled electronically.
The requested documents will be made available to the general public, and this request is not being made for commercial purposes.
