Pursuant to the Illinois Freedom of Information Act., I hereby request the following records:

I hope this request finds you in good spirits. I request that a copy of the following documents [or documents containing the following information] be provided to me that pertains to Menard Correctional Center:

* Number of staff whose use of force against inmates was found to be excessive (monthly, yearly).
* Title of staff whose use of force against inmates was excessive (monthly, yearly).
* Number of staff who were dismissed due to excessive use of force - or any similar reason - against inmates (monthly, yearly).
* Number of staff formally counseled, reprimanded, warned, or suspended (with or without pay) for excessive use of force - or physical, mental, verbal, and emotional abuse - monthly and yearly.
* Number of staff hired and related position title (monthly and yearly).
* Number of staff who quit (with or without notice) and reasons given for quitting (monthly and yearly).
* Length of employment for all wardens and COs as of June 2024, along with rank and badge number.
* Number of staff who were fired, dismissed, indicted, and prosecuted (for any reason) along with reasons, by month and year.

Please provide the records requested above for June 2023 to June 2024.

The requested documents will be made available to the general public, and this request is not being made for commercial purposes.
