Pursuant to the Indiana Access to Public Records Act, I hereby request the following records:
–A list of every hospital, clinic, or other institution that provides sexual assault forensic exams administered by a certified Sexual Assault Nurse Examiner (SANE).
The requested documents will be made available to the general public, and this request is not being made for commercial purposes.
In the event that there are fees, I would be grateful if you would inform me of the total charges in advance of fulfilling my request. I would prefer the request filled electronically, by e-mail attachment if available or CD-ROM if not.

Dear Ms. Nason:
On Oct. 18, 2017, the Indiana State Department of Health (ISDH) received your request for records pursuant to the Access to Public Records Act ("APRA"). Specifically, you requested:
* A list of every hospital, clinic, or other institution that provides sexual assault forensic exams administered by a certified Sexual Assault Nurse Examiner (SANE).
Indiana Code 16-21-8-1 states that "a hospital licensed under IC 16-21-2 that provides general medical and surgical hospital services shall provide forensic medical exams. ..." Every hospital providing these services would therefore be required to provide forensic medical exams. Under IC 16-21-8-3, the exams must be provided be either a physician or a Sexual Assault Nurse Examiner.
The Indiana State Department of Health does not differentiate between facilities in which a physician performs these exams and those where they are performed by a Sexual Assault Nurse Examiner and therefore has no records specific to your request. However, a list of hospitals licensed under IC 16-21-2 as described above can be found at http://www.in.gov/isdh/20123.htm.
