As stated in all responses back to you, you need to fill out a Public Request of Information form. I will attach one here so you can properly fill it out and fax it back to us. Upon completion of the request, you will be contacted to let you know what was approved, denied, and amount needing to be paid. Please make sure you provide a good contact number and address.
