Kindred Healthcare wage complaints

To Whom It May Concern:

Pursuant to the The Missouri Sunshine Law, I hereby request the following records:

Records of any and all wage complaints filed against Kindred Healthcare Inc. (also doing business as "Kindred Hospitals" or "Kindred At Home"), federal tax ID number 611323993,

for the inclusive period between January 1, 2010 and July 1, 2020,

with information including the date the complaint was filed, the nature of the complaint, the amount owed to the complainant, the dates worked but not compensated, and the outcome of the complaint.

The requested documents will be made available to the general public, and this request is not being made for commercial purposes.

In the event that there are fees, I would be grateful if you would inform me of the total charges in advance of fulfilling my request. I would prefer the request filled electronically, by e-mail attachment if available or CD-ROM if not.
