Use of Force Reports (2013-2016)

To Whom It May Concern:

Pursuant to the Kentucky Open Records Act, I hereby request the following records:

All use-of-force reports dating from the January 1, 2013 to December 31, 2016. In particular, I am looking for use-of-force reports which contain the following information: OFFICER(S) BADGE NUMBER, TIME OF OCCURRENCE (I.e., the HH:MM the use-of-force incident occurred), DATE RECEIVED, DATE OF OCCURRENCE, RECORD ID NUMBER, INCIDENT SUMMARY, TYPES OF FORCE USED (linked to each officer), SERVICE BEING RENDERED, REASON FOR USING FORCE, and ANY INFORMATION ON THE CITIZEN THAT MAINTAINS CONFIDENTIALITY (omit if unable to provide).

Please note the TIME OF OCCURRENCE is contained in this request.

The requested documents will be made available to the general public, and this request is not being made for commercial purposes.

In the event that there are fees, I would be grateful if you would inform me of the total charges in advance of fulfilling my request. I would prefer the request filled electronically, by e-mail attachment if available or CD-ROM if not.
