Traffic Accident Investigaiton report

To Whom It May Concern:

Pursuant to the Illinois Freedom of Information Act., I hereby request the following records:

All police reports and documents related to an investigation that involved a fatal car crash in which Neely Dotson (victim) died. The date of the accident was May 23, 2022 and it occurred on Lincoln Highway near Interstate 57 in the Village of Matteson. I am also requesting copies of any citations that were were given to the other driver in the accident, in addition to information that confirms the make and model of both vehicles involved in the crash. I am also requesting any police bodycam video that was taken in relation to this incident.

The requested documents will be made available to the general public, and this request is not being made for commercial purposes.

In the event that there are fees, I would be grateful if you would inform me of the total charges in advance of fulfilling my request. I would prefer the request filled electronically, by e-mail attachment if available or CD-ROM if not.
