Pursuant to the Florida Sunshine Law, I hereby request the following records:

I am requesting the complete case file on all deaths from January 1, 1981 to December 31, 1989 that contain "excited delirium" or "cocaine toxicity" or "cocaine intoxication" as a cause of death.

I am requesting scanned PDF copies of the complete case files including:
investigation report / scene investigation report
any hand written case information notes
notes of the classification of pending case
the classification of pending case page (which lists cause of death / manner of death, how incident occurred and the rationale for classification)
the autopsy report
the cause of death page
the complete toxicology report with results
the medical examiner's cover sheet describing date, location, age, address cause of death, and other preliminary findings

The requested documents will be made available to the general public, and this request is not being made for commercial purposes.

In the event that there are fees, I would be grateful if you would inform me of the total charges in advance of fulfilling my request. I would prefer the request filled electronically, by e-mail attachment if available or CD-ROM if not.
