Pursuant to the Michigan Freedom of Information Act, I hereby request the following records:
Historical licensing data for all licensed child care providers (active and closed) that were open anytime from 1/1/19-present:
1) Provider name or ID
2) Provider address
3) Latest license issue date and expiration date
4) First license issue date
5) Enrollment, including dates on which enrollment was recorded (by child age groups if available)
6) Capacity, including dates on which capacity was recorded (by child age groups if available)
7) Prices, including dates on which prices were recorded (by child age groups if available)
8) Quality Rating and Improvement System (QRIS) scores, including dates on which scores were recorded
9) Any additional provider data that your agency collects
If electronic records from before 1/1/19 are available, I’d be happy to collect them as well.
The requested documents will be made available to the general public, and this request is not being made for commercial purposes.
In the event that there are fees, I would be grateful if you would inform me of the total charges in advance of fulfilling my request. I would prefer the request filled electronically, by e-mail attachment if available or CD-ROM if not.
Thank you in advance for your anticipated cooperation in this matter. I look forward to receiving your response to this request within 5 business days, as the statute requires.

Thank you for following up on your FOIA request. MDE is in receipt of your 12/1/23 request. By FOIA statute<http://legislature.mi.gov/doc.aspx?mcl-15-235>, and MDE guidelines<https://www.michigan.gov/mde/resources/foia>, the 12/1/23 request was considered received the next business day, 12/4/23, and the department is allowed 5 business days to respond, as allowed by FOIA statute, which is 12/11/13. The department will respond to your requestion on or before the 11th.
