Pursuant to the Oklahoma Open Records Act, I hereby request the following records:
Any and all public records related to Incident #04-5812 (which was consolidated as part of Case Number CF-2004-4411), including initial offense reports, arrest records, jail registers, radio logs, crime summaries, and e-mail communications, as well as evidence inventory logs, property room general reports and collection forms, chain of custody forms, arrest reports, crime/incident reports and logs, and search warrants/affidavits.
The requested documents will be made available to the general public, and this request is not being made for commercial purposes.
In the event that there are fees, I would be grateful if you would inform me of the total charges in advance of fulfilling my request. I would prefer the request filled electronically, by e-mail attachment if available or CD-ROM if not.
