Pursuant to the Mississippi Public Records Act, I hereby request the following records:
Any and all CMS Form 2567s on file for the following home health agencies for the inclusive period between January 1, 2015 and September 29, 2020. In the event that there are multiple agencies with the same name or with almost identical names, I intend for this request to encompass all those agencies. LHC Group is a nationwide company that operates the other home care agencies as subsidiaries in the state of Mississippi.

The requested documents will be made available to the general public, and this request is not being made for commercial purposes.
