- Legal name (if different than commitment name)

- Birthdate
- Sex
- Race
- Last known address, city, state, and zip prior to custody
- Current facility location
- Current unit
- Location change date
- Entry date (or date of intake or start of custody)
- Number of sentences
- Total length
- For each sentence, offense details (including case number, offense code/statute, offense description, county of conviction, sentence length, sentence date)
- Tentative release date
The requested documents are for research purposes to contribute to public knowledge. This request is not being made for commercial purposes.
In the event that there are fees, I would be grateful if you would inform me of the total charges in advance of fulfilling my request. I would prefer the request filled electronically, by e-mail attachment if available or CD-ROM if not.
