Inmate Records Request

To Whom It May Concern:

Pursuant to the Arizona Public Records Act, I hereby request the following records:

This request covers multiple documents (from different booking dates) regarding the same inmate. Please read carefully to ensure that you provide a complete response.

Request #1: Please provide the Booking Information Form and Inmate Property Form from the Arrests of the Inmate on October 27th, 2020, April 11th, 2021, and May 8th, 2021.

Request #2: Please provide all images on file of the inmate from all arrests listed above. This should include the front, side, and any images taken of tattoos and/or markings on the inmate.

Request #3: Please provide all inmate kiosk requests and grievances that were filed by the below-listed inmate from May 8th, 2021 to June 7th, 2021.

Request #4: Please provide all discipline reports that were generated as a result of discipline issues with the inmate from May 8th, 2021 to June 7th, 2021. Please provide the full report, as well as any sanctions imposed.

Request #5: Please provide the full classification file from the May 8th, 2021 arrest of the inmate.
