Pursuant to the Illinois Freedom of Information Act., I hereby request the following records:

A log of all deaths recorded in Cook County in 2002, including but not limited to the following fields:
- First and last name of the deceased,
- Date of death,
- Date of birth,
- Address,
- Cause of death.

I preemptively agree to forego all sensitive private information such as social security numbers.

The requested documents will be made available to the general public, and this request is not being made for commercial purposes. I am a freelance reporter for Medill News and I am requesting this information in that editorial capacity.

In the event that there are fees, I would be grateful if you would inform me of the total charges in advance of fulfilling my request. I would prefer the request filled in the form of CSV or Excel file, and electronically by e-mail attachment if available.

Thank you in advance for your anticipated cooperation in this matter. I look forward to receiving your response to this request within 5 business days, as the statute requires.
