Please note that we have been unable to locate the original, signed copies of two of the PTAX-300, Application for Non-homestead Property Tax Exemption - County Board of Review Statement of Facts, forms. We have provided electronic copies in lieu of the originals. The IDOR docket numbers on these two applications are 14-016-00496 and 14-016-00537.

Please also note that private information, such as sales tax exemption number (E #s) and signatures, have been redacted pursuant to 5 ILCS 140/7(1)(b).
