Pursuant to the Idaho Public Records Act, I hereby request the following records:
A log of public records requests submitted to the Department of Health and Welfare that lists the date of request, the requester's name, the requester's organizational affiliation, a description of the request and the date the request was filled. Ideally I would like this information for the period from 2004 to the present.
The requested documents will be made available to the general public free of charge as part of the public information service at MuckRock.com, and is made in the process of academic research and not for commercial usage.
In the event that fees cannot be waived, I would be grateful if you would inform me of the total charges in advance of fulfilling my request. I would prefer the request filled electronically, by e-mail attachment if available or CD-ROM if not.

Pursuant to Section 9-339(1), Idaho Code<http://legislature.idaho.gov/idstat/Title9/T9CH3SECT9-339.htm>, employees of the Department have determined that they will need more than three (3) working days to retrieve or find the requested information.

Please see the attached letter which lists three (3) versions and the estimated cost per each. The Department staff have spent four (4) hours and 181 pages so far on this request (2 hrs and 100 pages will be deducted). These totals would be added to 'the dates & info requested' and 'all five fields' versions.

I would like to go with the 3-column option with date of request, information requested and the date filled. Let me know where to send the check for that item.
Also, when we spoke on the phone you mentioned having some tables already made with information on the types of public records your office has received from the past several years. Could you please send me that information as well?
