Pursuant to the New Hampshire Right to Know Law, I hereby request the following records:

1) The NH DMV "Request for Waiver of Personal Information" form [-- which is discussed in form DSMV490 [2] under the section "How do I request a waiver?"];
2) The current revision of form DSMV499 (titled "Request for Privacy of Personal Information"); and
3) All revisions of form DSMV499 from its first date of creation.

Currently I am unable to locate any of these forms on your website[1].
I have also attached an old revision of the DSMV499 form for reference.

I greatly appreciate your response (dated March 25, 2021) with the one (1) responsive document attached answering #2 of my public records request (dated January 27, 2021 and received on January 28, 2021).

Unfortunately, there is still confusion on my end as other responsive documents (#1 & #3) have not been received yet.

1. Still have not received the copy of the document/form titled NH DMV "Request for Waiver of Personal Information". As originally stated this form is mentioned by name within form DSMV490 under the section "How do I request a waiver?"

3. Still have not received form DSMV499 revision copies, but your response dated March 25, 2021 claims the attached DSMV499 with revision date of 01/02 is "the only version that is on file". Can your Agency's RAO please provide the appeal officer contact for your Agency?

Also, do you have a time frame for that?
