To Whom It May Concern:

Pursuant to North Carolina Public Records Law (G.S. §§ 132-1 through 132-10), I hereby request the following documents:

All records created and/or collected from January 1, 2015 to the present concerning bicycle-involved crashes in New Hanover County. This is to include but not be limited to:

1) information on the date and time of a collision;

2)the street, alley or “side street” where a collision occurred;

3) related crash reports and narratives;

4) information on resulting injuries and/or fatalities;

5) and information on the road users involved.

The requested documents will be made available to the general public, and this request is not being made for commercial purposes.

In the event that there are fees, I would be grateful if you would inform me of the total charges in advance of fulfilling my request. I would prefer the request filled electronically, by e-mail attachment if available or CD-ROM if not.

Thank you in advance for your anticipated cooperation in this matter. I would request your response within ten (10) business days.
