Pursuant to the New York Freedom of Information Law, I hereby request the following records:
For Andrew Kolodny, M.D., copies of all contracts, work and/or job descriptions, invoices, documentation of his work product/services rendered, records of numbers of hours worked and amounts paid to date for all services Kolodny has rendered as a vendor, consultant or for any other role in which he has been compensated by the state of New York
According to "Open Book New York," Kolodny has two recent contracts. They are contract T000491 and T000495. I do not know if there are other current or former contracts or employment records for Kolodny. Records for Kolodny may also exist under the name of the nonprofit he founded, Physicians for Responsible Opioid Prescribing (PROP).
The requested documents will be made available to the general public, and this request is not being made for commercial purposes.
In the event that there are fees, I would be grateful if you would inform me of the total charges in advance of fulfilling my request. I would prefer the request filled electronically, by e-mail attachment if available or CD-ROM if not.
Thank you in advance for your anticipated cooperation in this matter. I look forward to receiving your response to this request within 5 business days, as the statute requires.
