2. All ITS CISO Cyber Command Center Incident Notification Reports received by NYSOITS in the past two years
3. Data or spreadsheets on the number of ITS CISO Cyber Command Center Incident Notification Reports received by NYSOITS in the past two years, including the source of the reports
4. All copies of the New York State Security Breach Reporting Form sent to ciso@its.ny.gov in the past two years.
5. Data or spreadsheets on the number of New York State Security Breach Reporting Form received by ciso@its.ny.gov in the past two years, including the source of the reports
The requested documents will be made available to the general public, and this request is not being made for commercial purposes.
In the event that there are fees, I would be grateful if you would inform me of the total charges in advance of fulfilling my request. I would prefer the request filled electronically, by e-mail attachment if available or CD-ROM if not.
