Pursuant to the New York State Freedom of Information Law (1977 N.Y. Laws ch. 933), I hereby request the following records:
- a copy of the last DCA approved issued Catering Establishment license for the Bay Ridge Manor, indexed as License # 1303003-DCA
- all documents submitted by the Bay Ridge Manor for its most recent application for renewal of that license, indexed as Application # 10488-2016-RCAT
- all documentation of DCA's decision to deny the renewal request
- all documents related to disciplinary proceeding or penalties on account of previous violations, dating back to 2010.
The Bay Ridge Manor is located at 476 76 Street, Brooklyn, NY 11209
The requested documents will be made available to the general public, and this request is not being made for commercial purposes.
In the event that there are fees, I would be grateful if you would inform me of the total charges in advance of fulfilling my request. I would prefer the request filled electronically, by e-mail attachment if available or CD-ROM if not.
