Pursuant to the Ohio Open Records Law, I hereby request the following records:

All policies relating to, or mentioning, the word "ADA," "disability," "blind," or "deaf," or the phrase "Americans with Disabilities Act;"
The number of prisoners who have been identified by DOC as blind or as having visual impairments, if any;
The number of prisoners who have been identified by DOC as deaf or as being hard of hearing, if any;
The number of prisoners who have been identified by DOC as being blind and deaf, if any;
The number of prisoners who have been identified by DOC as having mobility impairments (e.g., spinal cord injuries, quadriplegia, etc.);
The number of prisoners placed in administrative segregation, protective custody, or similar status, and who have been identified by DOC as (1) blind or as having a visual impairment; (2) deaf or as being hard of hearing; (3) blind and deaf, and/or (4) having a mobility impairment;
All statistics concerning all requests for accommodation from offenders, and the nature and outcome of the accommodations requested, from January 2015-January 2016.
The number of ADA-related grievances by prisoners with disabilities that were filed from January 2015-January 2016;
The number of ADA-related grievances filed by prisoners with disabilities that were resolved from January 2015-January 2016; and
The number of ADA-related grievances filed by prisoners with disabilities that were pending as of January 2016.

The requested documents will be made available to the general public, and this request is not being made for commercial purposes.

We track this by number of inmates requiring a wheel chair and number of inmates with mobility issues but do not require a wheel chair:

Wheel chair: 338

Mobility issue: 830

Response is pending and will be forwarded as soon as received.

The number of ADA-related grievances by prisoners with disabilities that were filed from January 2015-January 2016;

Unable to query this data. However, we did conduct a review of grievances logged with the complaint code "handicapped facilities" and found 3 grievances filed.

The number of ADA-related grievances filed by prisoners with disabilities that were resolved from January 2015-January 2016; and

From the above number all were resolved

The number of ADA-related grievances filed by prisoners with disabilities that were pending as of January 2016.

All grievances are resolved within 14 days of receipt by the inspector unless there is an extension of an additional 14 days. There are no pending grievances from 2015

Good morning, please see the following as responsive to your inquiry. We do not keep stats regarding the nature of the request. Also, I note a request for records dated June 6, 2016 which appears to be a duplicate of the request via email dated March 24, 2016. Therefore, this response will be considered responsive to the June 6, 2016 request as well.
