Ohio IPHIS_EDRS Birth and Death Index

To Whom It May Concern:

Pursuant to the Ohio Public Records Act, Ohio Revised Code § 149.43, I am writing to request access to and a copy of the birth and death index from 1908 to the present day. Specifically, I am interested in obtaining a copy of the database from your Integrated Perinatal Health Information System and the Electronic Death Registration System (IPHIS-EDRS).

As stated in Ohio Revised Code § 149.43(B)(1), “all public records responsive to the request shall be promptly prepared and made available for inspection to any person at all reasonable times during regular business hours.” Moreover, the Ohio Revised Code defines public records broadly to include any document, device, or item, regardless of physical form or characteristic, created or received by or coming under the jurisdiction of any public office that documents the organization, functions, policies, decisions, procedures, operations, or other activities of the office.

Please inform me of any costs associated with fulfilling this request. I am willing to pay any reasonable fees for the processing of this request. If the cost exceeds $50, please contact me with an estimate before proceeding. However, if it is feasible, I would appreciate a fee waiver as this request is for the interest of the public and non-commercial educational purposes.

If my request is denied in whole or in part, please provide a written explanation for the denial, including the legal authority upon which the denial is based, as required by Ohio Revised Code § 149.43(B)(3).
