To Whom It May Concern:
Pursuant to the Ohio Open Records Law, I hereby request the following records:
Amount of TANF dollars allocated to the Ohio Parenting and Pregnancy Program, services and crisis centers and a list of corresponding agency recipients.
Any documents that outline the set plan for how this program is meeting at least one of the four federal requirements – “providing assistance to needy families, promoting job preparation and work, preventing and reducing out-of-wedlock pregnancies, and/or encouraging marriage and two-parent families”. I also request any reports produced by the Ohio Department of Job and Family Services Department that determine the completeness of the required elements of the federal law. This includes any notification sent to the Department from TANF funded crisis centers regarding policies changes.
Finally, I request any documents or reports generated by the Department that regard site visits to agencies that receive TANF funding through the Ohio Parenting and Pregnancy Program.
The timeframe for this request is FY2013 through FY2016.
The requested documents will be made available to the general public, and this request is not being made for commercial purposes.
In the event that there are fees, I would be grateful if you would inform me of the total charges in advance of fulfilling my request. I would prefer the request filled electronically, by e-mail attachment if available or CD-ROM if not.
Thank you in advance for your anticipated cooperation in this matter. I would request your response within ten (10) business days.
Sincerely,
Grace Raih
