Pursuant to the Oklahoma Open Records Act, I hereby request the following records:
Historical licensing data for all licensed child care providers (active and closed) that were open anytime from 1/1/19-present:
• Provider name or ID
• Provider address
• Latest license issue date and expiration date
• First license issue date
• Any additional provider data that your agency collects
If electronic records from before 1/1/19 are available, I’d be happy to collect them as well.
The requested documents will be made available to the general public, and this request is not being made for commercial purposes.
In the event that there are fees, I would be grateful if you would inform me of the total charges in advance of fulfilling my request. I would prefer the request filled electronically, by e-mail attachment if available or CD-ROM if not.

The Oklahoma Department of Human Services has received your request under the Oklahoma Open Records Act. This request has been forwarded to the appropriate individuals within the Department for a determination whether and what records may be in its possession, custody, or control. If responsive records are located, an assessment will be made whether any of those records fall within any exclusions contained within the Open Records Act itself. Any records that are responsive and do not fall within such exclusions will be provided.

On or about August 7, 2023, the Oklahoma Department of Human Services received a request for records under the Oklahoma Open Records Act (see below). Subsequent to a good faith, diligent search for records responsive to the parameters of the request, the Department discovered records were within its possession, custody, and control that it believes are responsive to this specific request. The records are attached.
The Department now considers this request closed.
