Pursuant to the Oklahoma Open Records Act, I hereby request the following records:

Scott Eizember's autopsy report, including but not limited to the amount of drugs that were found in his body, a description of his physical state post mortem, cause of death, any and all notes from a post-mortem examination.

Any and all notes, reports, emails, findings on the execution of Scott Eizember, from the personnel carrying it out, personnel witnessing it and personnel overseeing it.

Any and all recordings, audio or otherwise from preparation for the execution and during the execution of Scott Eizember.

The requested documents will be made available to the general public, and this request is not being made for commercial purposes.

In the event that there are fees, I would be grateful if you would inform me of the total charges in advance of fulfilling my request. I would prefer the request filled electronically, by e-mail attachment if available or CD-ROM if not.
