Pursuant to Pennsylvania Right to Know Act, I hereby request the following records:
(1) Copies of all awarded contracts for Contract #4400009722 including:
-copies of all vendor proposal
-copies of invoices or other commercial documents showing the amount of payment and/or credit received
-copies of settlement reports
-certificate of recycling, bills of lading or manifests
Time period of this request is 1/1/2015 - 1/28/2016.
(2) All emails including attachments that contain the keyword "4400009722". Time period of this request is 1/1/2015 - 1/28/2016.
The requested documents will be made available to the general public, and this request is not being made for commercial purposes.
In the event that there are fees, I would be grateful if you would inform me of the total charges in advance of fulfilling my request. I would prefer the request filled electronically, by e-mail attachment if available or CD-ROM if not.
